Requirements

   In order to complete the enrollment application folder the followings documents are required:

  1. Proof of Age ( Birth Certificate)
  2. Enrollment Application
  3. Previous school report card (if applicable)
  4. Medical History Form
  5. Copy of Immunization Records
  6. Copy of passport – (if applicable)
  7. Residence Card – (if applicable)
  8. 1 recent passport sized photo
  9. Student interview
  10. CSB placement test (if applicable)
  11. Registration fee received
